Baby Dedication Ceremonies are for infants and toddlers ages 0-3 years old.  If you wish to have your baby dedicated, please complete the form below.  IMPORTANT: If you have traveling family members and friends attending the ceremony, please wait until you receive an email confirmation prior to making any travel arrangements.
